Intake Paperwork

Intake paperwork is completely paperless and easy to complete. Prior to attending your first official session, you will receive an e-mail with login information to our client portal.  This portal is HIPAA (Health Insurance Portability and Accountability Act) compliant and encrypted to safely store your information.  A form of payment on file (credit, debit, health savings card) is required prior to attending a session. Please fill out all the required documentation and payment verification, which will allow us to move forward. By having your information and all the documentation filled out prior to session, we will only briefly review the paperwork when we meet, which means we can concentrate talking about your concerns and developing a treatment plan. 

You have the right to receive a "Good Faith Estimate" explaining how much your health care will cost. Under the law, healthcare providers need to give patients who don't have certain types of health care coverage or who are not using certain types of healthcare coverage an estimate of their bill for healthcare items and services before those items or services are provided.
